Limited Lines Credit Producer: Each individual whose duties will include selling soliciting, or negotiating limited lines credit insurance must complete an approved program of instruction to obtain a credit insurance license.

Licensing Directions – Self Study
1. Read and study the approved licensing material and complete the worksheet enclosed by circling the correct answers.
2. Complete the COMPLETION VERIFICATION form verifying material has been read/studied.
3. Mail the completed and signed COMPLETION VERIFICATION form, worksheet, and a self-addressed, stamped envelope to the following:

4. We will mail you a Certificate of Completion.
5. Mail a copy of the Certificate of Completion, completed application (pages 1-4), and a check for $40.00 to the following:
